LVL1 Wings fail at 100%

Hey guys!
I had 4 or 5 different combination fail when I tried to make a level 1 wing.
I use a chaos weapon +4 +opt, Jewel of chaos and lesser stones. I even put it Jewel of soul one time to make sure. 

The success rate is at 100% every single time but nothing. 
I made to much stones to have it at 100%. It's quite infuriating.
